Interested in participating?
 
Here is what we need from you to finalize your registration and payment no later than April 10th.

1) Complete the Registration Form at www.skateforkids.com and return it by e-mail to prendergast@smcsmail.com

2) Mail or courier a cheque for $2800 payable to Skate for Kids with a hard copy of the Registration Form to Stephen Milic, Mandrake Management Consultants,
401-55 St. Clair Avenue West, Toronto, Ontario, M4V 2Y7.

Note: The name on the cheque will be the name listed on the tax receipt for $1400. The payment can be made with a personal cheque (for a personal tax receipt) or a corporate cheque(for a corporate tax receipt).

Teams that register and pay their fee by April 10 will have an opportunity to choose one of their two NHL players for their roster(subject to availablity). The list of former NHL players will be available on the Skate for Kids website as of April 17.
